We are currently seeking a compassionate Optometrist to join our 16th Street Health Center. This full-time opportunity is ideal for someone passionate about visual health and health equity. You’ll deliver comprehensive vision care while collaborating with interdisciplinary teams to support whole-person health in a community-based setting.

Job Summary

As an Optometrist at JPCHC, you’ll provide routine and diagnostic eye care services for patients across the lifespan. From managing ocular disease to prescribing corrective lenses, your work will directly impact patients’ quality of life. You’ll be a key part of our integrated care model—collaborating with medical, dental, and behavioral health teams to ensure every patient receives the care they deserve.

Job Responsibilities (include but are not limited to):

  • Perform comprehensive eye exams, visual acuity tests, and diagnostic procedures.
  • Diagnose and manage conditions such as glaucoma, cataracts, macular degeneration, diabetic retinopathy, and refractive errors.
  • Prescribe glasses, contact lenses, and vision therapy.
  • Provide pre- and post-operative care in collaboration with ophthalmology.
  • Document findings and treatment plans thoroughly in the EHR system.
  • Educate patients and families on eye health, disease prevention, and corrective devices.
  • Refer patients for advanced or surgical care when appropriate.
  • Support vision-related community outreach, screenings, and health events.
  • Supervise optometric technicians and assist with workflow improvements.
  • Ensure compliance with FQHC, HIPAA, and infection control standards.

What You’ll Bring

  • Doctor of Optometry (OD) degree from an accredited school
  • Active, unrestricted license to practice optometry in Indiana (or ability to obtain)
  • 1–2 years of clinical experience preferred; strong new grads considered
  • Experience working in a community health or FQHC setting is a plus
  • Strong clinical, diagnostic, and communication skills
  • Commitment to culturally competent care for underserved populations

Additional Information

  • Moderate proficiency in Microsoft Office and EHR systems required
  • Must meet eligibility for credentialing and privileging
  • Ability to supervise clinical support staff and maintain infection control protocols
