Job Summary:

Under the supervision of the Enabling Services Senior Manager, the Social Services Coordinator will perform a variety of care management duties. Creates and maintains a high-performance environment characterized by positive leadership, patient oriented practices, and professionalism.

Responsibilities:

  • Connect patient to internal and external resources and services, including behavioral health, education, social services, financial, transportation, housing, food, safety, etc. Follow-up with patients to ensure successful referrals and problem-solve around unsuccessful referrals.
  • Evaluate and develop a plan of care personalized to fit the patient’s emotional and social needs.
  • Provide any appropriate action to obtain available community resources to assist in resolving medical or social issues.
  • Collaborate with providers to develop a care plan that addresses patient’s social, emotional, economic, and environmental needs as related to their healthcare.
  • Investigate, assess, and plan interventions to help patients cope with social, emotional, economic, and environmental problems.
  • Develop and maintain working relationships with community health, welfare, and social agencies and seek creative means to assist the client with their needs.
  • Frequent external contacts with patients/clients, families, health care, and social service agencies concerning referrals, care coordination; and acquisition/provision of confidential information.
  • Provide appropriate and high-quality feedback on patients identified for social and emotional support to the Behavioral Health Team.
  • Serves as a liaison between the patient and the health care provider concerning the broad psychosocial aspects of the patient’s course of treatment.
  • Expand outreach activities and referral networks to ensure effective partnerships to support the development of a comprehensive network of services.
  • Assist with food pantry as needed.
  • Provide valuable insight as related to the Social Determinants of Health.
  • Demonstrate the ability to independently exercise judgment on behalf of JPCHC.
  • Attend community meetings as a representative of JPCHC.
  • Participate in continuing education activities, remaining knowledgeable in areas of expertise and improving professional skills.
  • Provide safe, timely delivery of services.
  • Maintain HIPAA standards and ensure confidentiality of protected health information. Report critical incidents and information regarding quality-of-care issues.
  • Other duties as assigned and special projects as the manager sees fit.

Minimum Requirements:

  • Bachelor’s Degree in social work, psychology, or related field.
  • Community Health Worker certification preferred.
