Job Summary

Under the supervision of the Navigation Manager, the Discharge Coordination Navigator ensures seamless continuity of care by coordinating benefits and community resources for patients transitioning out of clinical settings. You will process benefit applications, conduct intake assessments, provide de-escalation support, and facilitate interdisciplinary care meetings—helping our patients bridge the gap between hospital discharge and ongoing support.

Job Responsibilities (include but are not limited to):

  • Benefit Application Support:
    • Process follow-up and provide hands-on assistance with HIP, Medicaid, TANF, and SNAP applications
    • Determine and submit presumptive eligibility paperwork
  • Intake & Assessment:
    • Conduct thorough intake assessments to identify individual navigation needs
    • Document findings and develop personalized service plans
  • Care Coordination:
    • Schedule and facilitate coordination meetings with therapists, the Nurse Manager, and the Navigation Manager
    • Maintain regular check-ins with the detox unit to monitor patient progress and identify resource gaps
  • Crisis & De-Escalation Support:
    • Provide de-escalation techniques to support patients in crisis
    • Refer patients to Indiana’s 24/7 crisis helplines and community behavioral health agencies
  • Patient Advocacy & Education:
    • Educate patients and families on available community resources and benefits
    • Advocate for timely access to services and follow through until enrollment is confirmed
  • Documentation & Reporting:
    • Maintain accurate, timely records in the electronic health record (EHR) system
    • Generate reports on application statuses, service gaps, and outcome metrics

Required Skills and Qualifications

  • Education & Certification:
    • High school diploma or equivalent required
    • Peer Recovery Certification preferred
    • Indiana Navigator License (or proof of eligibility) required within 90 days of hire
  • Experience:
    • 1–2 years’ experience in patient navigation, case management, or related social services
  • Core Competencies:
    • Strong working knowledge of HIP, Medicaid, TANF, SNAP, and presumptive eligibility processes
    • Excellent verbal and written communication skills
    • Highly organized with strong attention to detail
    • Ability to build rapport quickly and collaborate effectively across multidisciplinary teams
