Job Summary:

The LCAC Behavioral Health Clinician is responsible for the assessment, planning, implementation, coordination, monitoring, and evaluation of services across the continuum of care to ensure quality patient outcomes and appropriate utilization of health care services. The Behavioral Health Clinician is responsible for supporting the healthcare team towards a smooth transition from one level of care to another in support of the patient/family. The preferred candidate will have experience in substance use disorders and treatment.

Accepted licensures for this position: LCSW, LCAC, LMHC, LMFT

Group Facilitation Responsibilities:
The selected Addictions Therapist will be responsible for leading the following group sessions:

  • Relapse Prevention Group
    Tuesdays | 4:00 PM – 5:00 PM

  • Matrix Intensive Outpatient Program (IOP)
    Tuesdays, Wednesdays, and Thursdays | 5:30 PM – 7:30 PM

The therapist will provide structured, evidence-based support to individuals in recovery, ensuring a safe, engaging, and therapeutic group environment.

Summary of Essential Job Functions/Responsibilities:

  • Performs intake assessments, which include the completion of the initial mental status assessment, and psycho-social assessment.
  • Plans and evaluates individual service plans and monitors plan objectives.
  • Performs individual, family and group counseling utilizing approved treatment approaches as sanctioned by JPCHC.
  • Prepares patient progress reports and completion notices.
  • Assists patients with reaching their treatments goals.
  • Executes therapeutic interventions, skill building and case management for clients.
  • Completes chart documentation reviews that include psycho-social history, treatment planning, chart progress notes, quarterly reviews, and discharge summaries.
  • Maintains accurate records to ensure compliance with all Federal and State regulations.
  • Assists in mental health crisis situations.
  • Serves as an information resource for mental health issues.
  • Consults with physicians, therapists, and other medical professionals when appropriate.
  • Assess the psychological status of patients related to the patient’s illness and environment.
  • Assist patients with obtaining resources for prescription assistance, FMLA/Disability, housing applications, health education, crisis intervention, risk assessment and reduction and referrals to appropriate agencies.
  • Provide relevant referrals to outside agencies and service providers based upon needs.
  • Follow up with these referral sources on behalf of the patient.
  • Under the supervision of the Practice Manager, generate appropriate reports for quality and metrics on a monthly basis.
  • Provide information to patients or families and care givers and community agencies.
  • Provide patient and family-focused therapeutic crisis counseling.
  • Maintain clinical records on all patients referred to social work.

Education and/or Experience:

  • Master's Degree required.
  • CPI (Non-Violent Crisis Intervention) certification preferred.
  • Experience with the treatment of substance use disorders strongly preferred.
  • LCSW, LCAC, LMHC, LMFT licensure required.
  • Requires relevant health care, mental health or community-based experience.
  • Requires knowledge of the impact of the disease process and of human behavior to provide psychosocial assessments, short-term therapeutic counseling, and crisis intervention to patients, families and staff.
  • Requires knowledge of state and county adult and child protective legal mandates.
  • Requires knowledge of the principles of growth and development over the life span.
  • Maintains principles, practices and techniques of crisis intervention and de-escalation techniques.
  • Develop and maintain strong working relationships with the community, medical teams, law enforcement, adult and child protective services.
  • Requires proficiency in Microsoft Excel and Word.
  • Experience collaborating with physicians, NP, etc. as part of interdisciplinary team
