Job Summary: Licensed Clinical Social Worker (LCSW):
The Licensed Clinical Social Worker (LCSW) is responsible for the assessment, planning, implementation, coordination, monitoring, and evaluation of services across the continuum of care to ensure quality patient outcomes and appropriate utilization of health care services. The Licensed Clinical Social Worker (LCSW) is responsible for supporting the healthcare team towards a smooth transition from one level of care to another in support of the patient/family.
Required Education and/or Experience for Licensed Clinical Social Worker (LCSW):
- Master's Degree in Social Work required.
- LCSW, LCAC, LMHC, or LMFT licensure required.
- CPI (Non-Violent Crisis Intervention) certification preferred.
- Requires relevant health care, mental health or community-based experience.
- Requires knowledge of the impact of the disease process and of human behavior to provide psychosocial assessments, short-term therapeutic counseling, and crisis intervention to patients, families and staff.
- Requires knowledge of state and county adult and child protective legal mandates.
- Requires knowledge of the principles of growth and development over the life span.
- Maintains principles, practices and techniques of crisis intervention and de-escalation techniques.
- Develop and maintain strong working relationships with the community, medical teams, law enforcement, adult and child protective services.
- Experience collaborating with physicians, NP, etc. as part of interdisciplinary team
